A unique opportunity is available for Nurse Practitioners skilled in home health to perform comprehensive risk assessments for Medicare Advantage members throughout Washington County, MS. This per diem role offers true flexibility, allowing you to set your own schedule and work in communities such as Greenville, Leland, Hollandale, and nearby areas. Each assessment typically requires about 45 minutes and varies from annual check-ins to post-discharge visits.

Schedule:

  • Flexible shifts, Monday through Sunday
  • Self-scheduled hours; design a workweek that fits your lifestyle
  • Opportunity to commit to minimum weekly or bi-weekly hours as desired

Key Responsibilities:

  • Conduct in-home health risk assessments for Medicare Advantage members
  • Gather and review detailed medical histories, including conditions and medications
  • Identify gaps in care and offer recommendations to optimize patient health
  • Provide education to members on health conditions and medication adherence
  • No requirement to develop treatment plans or prescribe medications

Desired Qualifications:

  • Active Nurse Practitioner license in Mississippi
  • National Board Certification through AANP or ANCC
  • Graduated with an MSN from an accredited nursing school
  • Current CPR (BLS) certification
  • At least six months recent NP experience preferred

This role includes competitive compensation ranging from $80 to $90 per evaluation, with additional stipends provided for mileage and administrative tasks.
