Make a meaningful impact by delivering comprehensive healthcare directly to patients homes as an experienced Nurse Practitioner in home health. This per diem opportunity involves conducting risk assessments for Medicare Advantage plan members throughout Raleigh County, WV, including Beckley, Mabscott, Sophia, and nearby areas. Assessments typically take 45 minutes and may be performed as part of annual evaluations or following hospital discharges.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Choose a minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete in-home annual health risk assessments for Medicare Advantage members
- Review and evaluate patients medical histories, current conditions, and medications
- Offer education on chronic and acute health concerns as well as prescribed medications
- Identify care gaps and suggest actionable solutions to improve patient outcomes
- No requirement to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in West Virginia
- Current BLS/CPR certification
- Board certification via AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited program
- At least six months experience as a Nurse Practitioner preferred
This role offers compensation of $80-$90 per completed evaluation, along with a mileage stipend and paid administrative time, providing both flexibility and competitive earning potential.
