Schedule:

- Meet minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Perform comprehensive annual and post-discharge home health risk assessments on Medicare Advantage plan members in their residences
- Evaluate members prior and current medical history, health conditions, and medication use
- Identify gaps in care and offer clear recommendations to help patients improve their health outcomes
- Educate members about their medical conditions and prescribed therapies
- No requirement to generate formal treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Board Certification through AANP or ANCC
- Current BLS (CPR) certification
- MSN degree from an accredited nursing program
- At least six months of current NP experience preferred
