Step into a pivotal role supporting patient health and well-being as a Nurse Practitioner conducting home health risk assessments for Medicare Advantage members throughout Cocke County, TN. This flexible, per diem opportunity is designed for compassionate Nurse Practitioners looking for autonomy and meaningful, face-to-face patient care.

Schedule:

• Flexible shifts available Monday through Sunday
• Set your own schedule based on your availability
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Complete annual health risk assessments for Medicare Advantage plan members residing in areas like Newport and Parrottsville
• Evaluate patients prior and current medical histories, conditions, and medication regimens
• Educate patients on the management of medical conditions and proper medication use
• Identify and report any gaps in care, recommending interventions to address these needs
• Focus solely on assessmentno prescriptions or treatment plans required
• Each in-home risk assessment typically takes around 45 minutes

Qualifications:

• Valid Nurse Practitioner license in Tennessee
• Board Certification through AANP or ANCC
• MSN degree from an accredited nursing program
• Current CPR certification (BLS)
• Minimum of six months Nurse Practitioner experience preferred
