Exceptional opportunities are available for licensed Nurse Practitioners interested in making a direct impact on patient health through home health risk assessments. As part of a per diem team, you will visit Medicare Advantage members in various areas across Cocke County, including Newport and Parrottsville, conducting comprehensive evaluations while enjoying the flexibility of creating your own work schedule.
Schedule:
- Flexible shifts available seven days a week
- Set your own hours
- Minimum weekly or bi-weekly hourly commitment
Primary Responsibilities:
- Perform thorough annual and post-discharge risk assessments in patients homes
- Review and evaluate patients prior and current medical histories, conditions, and medications
- Educate patients about their health conditions and medication regimens
- Identify gaps in healthcare and recommend actionable solutions
- Focus on assessmentno need for treatment planning or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Master of Science in Nursing (MSN) from an accredited program
- Current BLS CPR certification
- Board certification via AANP or ANCC
- At least 6 months of recent Nurse Practitioner experience preferred
