A valuable opportunity is available for a skilled Nurse Practitioner with home health experience to provide comprehensive risk assessments for Medicare Advantage plan members in Cayuga County. This per diem position offers flexibility and autonomy while supporting clients in locations such as Auburn, Cayuga, Union Springs, and Moravia. The roles primary focus is on risk evaluation rather than ongoing care management, making it ideal for professionals seeking independent, rewarding work.

Schedule:

  • Flexible shifts available seven days a week
  • Create your own schedule
  • Minimum weekly or bi-weekly hourly commitment required

Responsibilities:

  • Perform thorough home health risk assessments for Medicare Advantage members.
  • Spend approximately 45 minutes per evaluation, providing focused, high-quality assessments.
  • Review patients prior and current medical histories, health conditions, and medication use.
  • Educate individuals on managing medical conditions and prescribed treatments.
  • Identify care gaps and recommend appropriate next steps.
  • Visits may include annual health assessments and post-discharge check-ins.
  • No prescribing or creation of treatment plans required.

Qualifications:

  • Valid Nurse Practitioner license in New York State.
  • Board Certified through AANP or ANCC.
  • Current Basic Life Support (BLS/CPR) certification.
  • MSN degree from an accredited nursing program.
  • At least six months of recent Nurse Practitioner experience preferred.

Compensation: Earn $80$90 per evaluation, with an additional mileage stipend and compensation for administrative time.
