Job Description

Take on an impactful opportunity delivering essential care in the community as a Nurse Practitioner providing home health risk assessments. In this per diem position, youll perform visits for Medicare Advantage plan members throughout Cayuga County, NYincluding locations such as Auburn, Cayuga, Union Springs, and Moravia. Visits are scheduled flexibly, allowing you to design your own workweek while making a meaningful difference in patients lives.

Schedule:

  • Flexible shifts available seven days a week
  • Self-selected work schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform home health risk assessmentseach averaging about 45 minutesfor Medicare Advantage members
  • Review and evaluate patient medical histories, current health conditions, and medication usage
  • Provide education to patients on diagnoses and medication adherence
  • Identify gaps in care and offer recommendations for follow-up or resolution
  • Focus is solely on assessmentno requirement to prescribe medication or create ongoing treatment plans

Qualifications:

  • Current Nurse Practitioner license in NY
  • Certification in Basic Life Support (CPR/BLS)
  • Board Certification via AANP or ANCC
  • A minimum of six months' NP experience (preferred)
  • MSN degree from an accredited nursing school

Compensation and Perks:

  • Earn $80-$90 per evaluation
