Job Description

Make a direct impact on community health by conducting essential home health risk assessments for Medicare Advantage plan members throughout Seminole County, OK. This per diem opportunity empowers Nurse Practitioners to create their own work schedules, offering flexibility and autonomy. Typical visits average 45 minutes and may include annual health assessments or post-discharge evaluations. You may be scheduled in various Seminole County locations such as Wewoka, Bowlegs, and Konawa during your shifts.

Schedule:

  • Flexible shifts available between Monday and Sunday
  • Create your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct annual home health risk assessments for Medicare Advantage plan members
  • Review and evaluate prior and current medical history, medical conditions, and medication history
  • Educate patients on disease processes, medications, and overall health management
  • Identify gaps in care and offer actionable recommendations to resolve them
  • No treatment plan creation or medication prescribing required

Qualifications:

  • Active Nurse Practitioner license in Oklahoma
  • Current BLS/CPR certification
  • Board Certification through AANP or ANCC required
  • Master of Science in Nursing (MSN) from an accredited institution
  • Minimum of 6 months current Nurse Practitioner experience strongly preferred
