This role focuses on conducting comprehensive home health risk assessments for Medicare Advantage plan members throughout Pinellas County, FLincluding St. Petersburg, Clearwater, and Largo. Each evaluation is designed to enhance patient outcomes while supporting healthcare continuity outside traditional clinical settings.

Schedule:

• Create your own work schedule
• Flexible shifts available Monday through Sunday
• Commit to a minimum number of shifts on a weekly or bi-weekly basis

Key Responsibilities:

• Perform annual home health risk assessments in members residences
• Review prior and current medical history, chronic conditions, and medication profiles
• Provide informative guidance to patients about health conditions and medication management
• Identify and communicate care gaps, recommending solutions as appropriate
• No requirement to create treatment plans or prescribe medications

Required Qualifications:

• Active Nurse Practitioner license in Florida
• Current certification in Basic Life Support (CPR/BLS)
• Board certification from AANP or ANCC
• At least 6 months of recent Nurse Practitioner experience preferred
• Masters degree in Nursing (MSN) from an accredited institution

In return for your clinical expertise and commitment, youll receive compensation of $80$90 per completed evaluation, plus a mileage stipend and additional pay for administrative tasks.
