Elevate patient care as a Nurse Practitioner supporting Medicare Advantage members through in-home health risk assessments in Pinellas County, Florida. This per diem opportunity offers autonomy and flexibility, allowing professionals to craft a schedule that fits their lifestyle and meet minimum weekly or bi-weekly hour commitments.

Schedule:

• Flexible shifts available Monday through Sunday
• Choose your own schedule
• Minimum hourly commitments on a weekly or bi-weekly basis

Responsibilities:

• Complete comprehensive in-home health risk assessments (approximately 45 minutes per visit) for Medicare Advantage plan members
• Review patients' prior and current medical histories, conditions, and medications
• Educate patients on their diagnoses and medication regimens
• Identify and document gaps in care, recommending corrective actions as needed
• No involvement in treatment plan development or medication prescribing required

Required Qualifications:

• Active Nurse Practitioner license in Florida
• Board certification via AANP or ANCC
• MSN degree from an accredited institution
• Current BLS (CPR certification)
• At least six months of recent Nurse Practitioner experience preferred
