Advance your expertise as a Nurse Practitioner specializing in home health by conducting comprehensive risk assessments for Medicare Advantage members in Sussex County, DE. This per diem role offers the flexibility to create your own schedule, with shifts available any day of the week. Visits take place in patients homes throughout communities such as Seaford, Milford, and Georgetown, and may involve either annual health assessments or post-discharge evaluations.
Schedule
- Flexible hours; design your own shifts Monday through Sunday
- Choose minimum weekly or bi-weekly hourly commitments to suit your availability
Primary Responsibilities
- Perform annual risk assessments for Medicare Advantage health plan members in their homes
- Review patients medical records, current medical conditions, and medication lists
- Educate patients on their diagnoses and prescribed medications
- Recognize and document care gaps, providing recommendations for follow-up or resolution
- Focus exclusively on assessmentsno creation of treatment plans or prescribing required
Qualifications
- Active Nurse Practitioner license in Delaware
- Board certification via AANP or ANCC
- CPR certification (BLS)
- At least six months of recent NP experience is preferred
- MSN from an accredited nursing school
