Make a meaningful impact by conducting important home health risk assessments for Medicare Advantage members in Cape Girardeau County and nearby communities. In this per diem role, youll utilize your Nurse Practitioner expertise to support patients by completing individualized assessments in their homesoften within convenient travel distance throughout areas like Jackson, Dutchtown, and Millersville. This is an excellent opportunity to exercise your clinical skills while enjoying flexible scheduling that works for you.

Schedule:

• Flexible shifts available any day of the week, Monday through Sunday
• Self-schedulingchoose your own shifts
• Minimum commitment on a weekly or bi-weekly basis

Key Responsibilities:

• Perform comprehensive annual and post-discharge home health risk assessments for Medicare Advantage members
• Review and document past and current medical histories, conditions, and medication use
• Educate members and families about ongoing conditions and prescribed medications
• Recognize care gaps and recommend next steps; no need for prescribing medications or creating treatment plans

Qualifications:

• Active Missouri Nurse Practitioner license
• Board Certification from AANP or ANCC
• MSN from an accredited nursing school
• Current CPR certification (BLS)
• At least 6 months of recent Nurse Practitioner experience preferred
