Job Description

Take on a rewarding role delivering personalized care as a Nurse Practitioner conducting home health risk assessments for individuals enrolled in Medicare Advantage plans. This per diem opportunity offers flexible scheduling and the ability to make a meaningful impact by providing service throughout Rensselaer County, including Troy, Rensselaer, and Hoosick Falls. Assessments average 45 minutes and vary by visit type, such as annual assessments or post-discharge visits.

Schedule:

  • Flexible shifts available between Monday and Sunday
  • Option to set your own schedule
  • Commitment to a minimum number of hours weekly or bi-weekly

Key Responsibilities:

  • Perform comprehensive home health risk assessments for Medicare Advantage members
  • Review and evaluate medical histories, current conditions, and medications
  • Educate individuals about their health conditions and medication plans
  • Identify gaps in care and recommend solutions for improved health management
  • No need to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in New York
  • Current CPR certification (BLS)
  • Board certified via AANP or ANCC
  • At least 6 months of recent Nurse Practitioner experience preferred
  • MSN from an accredited nursing school
