Take the next step in your advanced practice career as a Nurse Practitioner specializing in home health assessments. This per diem opportunity offers the flexibility to create your own schedule and deliver care to Medicare Advantage members across Saratoga Countyincluding Saratoga Springs, Clifton Park, and Halfmoon.
Schedule:
- Flexible shifts available Monday through Sunday
- Choose your own working days and hours
- Commit to weekly or bi-weekly shifts as your availability allows
Key Responsibilities:
- Conduct comprehensive home health risk assessments, each typically lasting 45 minutes
- Evaluate patients current and prior medical history, review current medications, and assess chronic and acute conditions
- Educate patients about their diagnoses, conditions, and medication regimens
- Identify care gaps and deliver actionable recommendations to help improve patient outcomes
- Focus solely on assessmentno requirement for ongoing treatment plans or prescriptions
Qualifications:
- Valid Nurse Practitioner license in New York State
- Board Certification as a Nurse Practitioner through AANP or ANCC
- Current Basic Life Support (BLS/CPR) certification
- At least 6 months of recent Nurse Practitioner experience (preferred)
- Master of Science in Nursing (MSN) from an accredited institution
This position offers a competitive compensation per completed evaluation, with additional stipends for mileage and administrative time.
