Job Description

Make a meaningful impact on patient well-being by providing comprehensive in-home health risk assessments for Medicare Advantage plan members across Saratoga County, NYincluding Saratoga Springs, Clifton Park, and Halfmoon. This per diem role offers the unique opportunity to work autonomously, use your clinical expertise, and deliver compassionate assessment-based careall with the flexibility to design your own schedule.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create appointments around your availability
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Perform annual or post-discharge home health risk assessments, each approximately 45 minutes in length
  • Review prior and current medical history, existing conditions, and medication profile
  • Offer patient education on health issues and prescribed medications
  • Identify gaps in care and recommend strategies or follow-up actions
  • No requirement to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner License in New York State
  • Current BLS CPR certification
  • Board certification via AANP or ANCC
  • Minimum of 6 months recent nurse practitioner experience preferred
  • Masters degree in Nursing (MSN) from an accredited university
