Job Description

Step into a critical role supporting the health and wellbeing of Medicare Advantage members by conducting home health risk assessments in Schenectady County, NY. This per diem opportunity empowers Nurse Practitioners to perform essential evaluations in a flexible, autonomous environment, making a meaningful impact on patient lives throughout the area.

Schedule:

Key Responsibilities:

  • Perform comprehensive annual risk assessments in member homes, each generally lasting about 45 minutes
  • Assess medical history, chronic and current health conditions, and medication usage
  • Educate patients on their conditions and medications, ensuring clear understanding
  • Identify gaps in care, offering actionable recommendations for improvement
  • Visits may include routine annual assessments or post-discharge evaluations
  • No requirement to develop treatment plans or prescribe medicationfocus is strictly on assessment and education

Desired Qualifications:

  • Active Nurse Practitioner license in New York State
  • Current CPR certification (BLS)
  • Board Certification via AANP or ANCC
  • Minimum of 6 months NP experience preferred
  • Master of Science in Nursing (MSN) from an accredited institution
