Job Description
Deliver essential healthcare services as a Nurse Practitioner specializing in home health risk assessments. This per diem opportunity offers the flexibility to design your own schedule, with shifts available any day of the week. You will serve individuals throughout Schenectady County by conducting comprehensive health assessments within patients homes.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own working hours based on weekly or bi-weekly commitments
Key Responsibilities:
- Conduct annual home health risk assessments for Medicare Advantage plan members
- Review and evaluate medical histories, ongoing conditions, and medication usage
- Educate patients about their current health conditions and prescribed medications
- Identify gaps in patient care and provide actionable recommendations
- No expectations to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in the state of New York
- MSN degree from an accredited nursing program
- Board certification through AANP or ANCC
- Current CPR certification (BLS required)
- At least 6 months experience as a practicing NP preferred
