Enjoy the flexibility of per diem work as a Nurse Practitioner specializing in home health risk assessments. This role offers the opportunity to provide valuable care to members enrolled in Medicare Advantage plans throughout Rensselaer County, NYincluding Troy, Rensselaer, and Hoosick Falls. Youll conduct comprehensive in-home evaluations, each averaging about 45 minutes, assisting patients with annual assessments or post-discharge needs.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own schedule to match your availability
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage plan members
- Evaluate medical history, current conditions, and medication regimens
- Educate patients about their health conditions and medication management
- Identify care gaps and make relevant recommendations to bridge these gaps
- No requirement for creating treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in NY
- Board Certified through AANP or ANCC
- Current CPR/BLS certification
- Master of Science in Nursing (MSN) from an accredited nursing program
- A minimum of 6 months recent NP experience preferred
This per diem opportunity provides a competitive rate of $80-$90 per completed evaluation, along with mileage reimbursement and compensation for administrative time.
