Job Description

Enjoy flexible, per diem scheduling that fits your lifestyle while providing important evaluations for Medicare Advantage plan members throughout Rensselaer County, including Troy, Rensselaer, and Hoosick Falls. Leverage your clinical expertise in varied community settings, conducting focused assessments and offering education that empowers patients.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling: set your own hours
  • Weekly or bi-weekly minimum hourly commitment required

Key Responsibilities:

  • Complete annual home health risk assessments for Medicare Advantage members
  • Review prior and current medical history, medical conditions, and medication use
  • Educate patients on their health conditions and medications
  • Identify and address gaps in care, providing recommendations as appropriate
  • Focus solely on assessments; no prescribing or treatment planning needed
  • Each evaluation averages 45 minutes in members homes

Qualifications:

  • Active Nurse Practitioner license in New York
  • Board certification from AANP or ANCC
  • Current BLS (CPR) certification
  • Graduation from an accredited MSN program
  • At least 6 months of recent NP experience preferred
