Job Description

As a Nurse Practitioner providing home health risk assessments on a per diem basis, youll have the unique opportunity to work with Medicare Advantage plan members throughout Albany County, including areas such as Cohoes, Watervliet, and Westmere.

Schedule:

  • Flexible shifts available seven days a week
  • Option to create your own schedule
  • Commitment based on your availabilityweekly or bi-weekly

Qualifications:

  • Active Nurse Practitioner license in New York
  • Current BLS (CPR) certification
  • Board certification through AANP or ANCC
  • Master of Science in Nursing (MSN) from an accredited school
  • Six months of recent NP experience preferred

Responsibilities:

  • Conduct comprehensive annual health risk assessments, with each visit averaging approximately 45 minutes
  • Review and evaluate patient health histories, ongoing conditions, and medication usage
  • Educate patients about their health conditions and prescribed medications
  • Identify any gaps in care and make practical recommendations to enhance overall health
  • No requirement to develop treatment plans or prescribe medications

As part of this engaging per diem role, you will receive competitive pay of $80$90 per evaluation, as well as a mileage stipend and compensation for administrative time.
