Exciting opportunity available for a Nurse Practitioner seeking per diem assignments in the evolving field of home health care. This position focuses on conducting comprehensive risk assessments for Medicare Advantage plan members across multiple locations within McHenry County, including Crystal Lake, Algonquin, and Lake in the Hills. Schedule:

  • Flexible shifts (Monday through Sunday)
  • Create your own schedule
  • Minimum hourly commitment weekly or bi-weekly

Key Responsibilities:

  • Complete annual home health risk assessments for Medicare Advantage members in their homes
  • Evaluate both prior and ongoing medical history, current conditions, and medication usage
  • Educate patients on managing medical conditions and understanding medications
  • Identify gaps in care, making thoughtful recommendations for patient health improvements
  • Visits average about 45 minutes and may include annual assessments or post-discharge check-ins
  • No initiation of treatment plans or medication prescribing required

Qualifications:

  • Active Nurse Practitioner License in Illinois
  • Current CPR certification (BLS)
  • Board Certification (AANP or ANCC)
  • MSN degree from an accredited school of nursing
  • At least 6 months of recent NP experience preferred, but all qualified candidates considered
