Advance your career as a Nurse Practitioner in a per diem role focusing on home health within Beckham County, OK. This flexible opportunity allows you to create your own schedule while supporting Medicare Advantage plan members living in Elk City, Sayre, Erick, and nearby areas. The position centers on conducting home health risk assessmentseach typically lasting around 45 minutesperformed during annual health checks or following hospital discharges. Scheduling may include any of these locations during your shift, providing a dynamic work experience.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling to fit your availability
- Minimum hourly commitment required weekly or bi-weekly
Key Responsibilities:
- Perform annual health risk evaluations in clients homes for a Medicare Advantage plan
- Assess prior and current medical history, ongoing conditions, and medication use
- Offer education about disease management and prescriptions to promote understanding
- Identify gaps in care and recommend actionable solutions to enhance patient outcomes
- Note: This role does not require the creation of treatment plans or the prescribing of medications
Qualifications:
- Active Nurse Practitioner license in Oklahoma
- Board Certification via AANP or ANCC
- Current CPR (BLS) certification
- MSN degree from an accredited nursing program
- At least 6 months of recent experience as an NP preferred
Compensation for this role includes $80$90 per evaluation as well as a mileage stipend and pay for administrative time.
