Job Description
Seeking a skilled Nurse Practitioner to conduct home health risk assessments on a per diem basis for Medicare Advantage plan members throughout Greene County, NY. This flexible opportunity allows you to provide meaningful care in the comfort of patients homes, focusing on comprehensive health reviews without treatment planning or prescribing responsibilities. You will work with a diverse population, serving locations such as Catskill, Cozsackie, Ashland, and Ravena, ensuring impact across the wider community.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own schedule to fit your availability
- Choose your weekly or bi-weekly hourly commitment
Responsibilities:
- Perform annual health risk assessments, typically 45 minutes per visit
- Review and evaluate patients' prior/current medical history, ongoing medical conditions, and medication use
- Educate patients about their health status, medical conditions, and prescribed medications
- Identify care gaps and provide appropriate recommendations to ensure quality and continuity of care
- No development of treatment plans or medication prescribing required
Qualifications:
- Active Nurse Practitioner license in New York
- Current CPR certification (BLS)
- AANP or ANCC board certification
- MSN degree from an accredited nursing program
- At least 6 months recent NP experience (preferred)
