Step into a dynamic per diem opportunity as a Nurse Practitioner specializing in home health risk assessments. This role empowers you to make a positive impact on Medicare Advantage plan members within Putnam County, Florida, including Palatka, Interlachen, and Crescent City. Ideal for clinicians who appreciate variety, flexibility, and direct patient interaction, each evaluation averages 45 minutes and may consist of annual health assessments or post-discharge visits.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own work schedule
  • Set your minimum hourly commitment on a weekly or bi-weekly basis

Key Responsibilities:

  • Complete comprehensive annual home health risk assessments for Medicare Advantage members
  • Assess prior and current medical conditions, review medication history, and document relevant findings
  • Educate patients on their health conditions and prescribed medications
  • Identify potential gaps in care and suggest actionable solutions to enhance patient outcomes
  • No requirement to establish treatment plans or prescribe medications

Qualifications:

  • Valid Nurse Practitioner license in the State of Florida
  • Active BLS (CPR) certification
  • Board Certification through AANP or ANCC
  • Minimum of six months of current Nurse Practitioner experience preferred
  • Master of Science in Nursing (MSN) from an accredited program

This position offers competitive earnings of $80 to $90 per evaluation, supplemented by a mileage stipend and compensation for administrative time.
