Deliver meaningful care and make a difference in the lives of Medicare Advantage plan members in Putnam County, FL as a skilled Nurse Practitioner specializing in home health. This per diem position offers unmatched flexibility, allowing you to create your own schedule and choose shifts between Monday and Sunday with a minimum weekly or bi-weekly hourly commitment.
Key Responsibilities:
- Conduct comprehensive home health risk assessments for Medicare Advantage members in locations such as Palatka, Interlachen, Crescent City, and other areas within Putnam County
- Perform detailed evaluations of patients medical history, current conditions, and medications
- Educate patients and caregivers about medical concerns, disease management, and prescribed therapies
- Identify, document, and communicate gaps in healthcare or wellness
- Offer clinical recommendations to support care coordinationwithout responsibility for making treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Florida
- Board Certification through AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited institution
- Current CPR certification (BLS)
- At least 6 months of recent Nurse Practitioner experience is preferred
- Strong interpersonal and communication skills; ability to work independently in a home health setting
This role provides competitive compensation, with $80$90 for each assessment completed, a stipend for mileage, and paid administrative time.
