Key Responsibilities:

• Perform comprehensive home health risk assessments (approx. 45 minutes each) in members residences within Saratoga County, including Saratoga Springs, Clifton Park, and Halfmoon
• Assess prior and current medical history, review medical conditions, and evaluate medication use
• Deliver essential health education related to conditions and medications
• Spot gaps in care and provide clear recommendations for follow-up
• No treatment planning or prescription writing involved

Qualifications:

• Active Nurse Practitioner license in New York
• Board Certification via AANP or ANCC
• Current Basic Life Support (BLS/CPR) certification
• MSN from an accredited nursing institution
• 6+ months of nurse practitioner experience preferred
