Job Description

Take on an impactful role providing in-home healthcare services as a Nurse Practitioner with a focus on home health risk assessments. This per diem position allows you to make a difference in the lives of Medicare Advantage plan members throughout Saratoga Countyincluding Saratoga Springs, Clifton Park, and Halfmoon. Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule around your availability
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Conduct comprehensive annual and post-discharge home health risk assessments for Medicare Advantage members
  • Evaluate prior and current medical history, medical conditions, and medication histories during each assessment
  • Provide patient education regarding ongoing medical conditions and medications
  • Identify gaps in care and make clear recommendations for further action or follow-up
  • Enjoy a patient-centered approach without developing treatment plans or prescribing medication

Qualifications:

  • Active Nurse Practitioner license in New York State
  • Board Certification via AANP or ANCC
  • Current CPR certification (BLS)
  • Minimum of 6 months experience practicing as a Nurse Practitioner preferred
  • MSN degree from an accredited nursing program
