Job Description

Leverage your clinical expertise as a Nurse Practitioner in a flexible per diem position focused on delivering high-quality home health risk assessments for Medicare Advantage members throughout Saratoga County. This role allows you to create your own schedule, working shifts that suit your lifestyle and commitments, with opportunities available across several communities including Clifton Park, Saratoga Springs, and Halfmoon.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling to fit your personal and professional needs
  • Minimum hourly commitment on a weekly or bi-weekly basis

Key Responsibilities:

  • Perform comprehensive annual health risk assessments in patients homes, typically requiring about 45 minutes per visit
  • Review and evaluate patients medical histories, existing health conditions, and medication regimens
  • Educate members on their medical conditions and appropriate medication management
  • Assess for gaps in care and make actionable recommendations to promote health and prevent complications
  • Focus exclusively on assessmentsno ongoing treatment planning or prescribing required

Qualifications:

  • Current, active Nurse Practitioner license in New York State
  • Board certification via AANP or ANCC
  • Basic Life Support (BLS/CPR) certification
  • At least 6 months of recent experience as an NP (preferred)
  • Master of Science in Nursing (MSN) from an accredited program
