Take on an impactful role providing in-home healthcare services as a Nurse Practitioner with a focus on home health risk assessments. This per diem position allows you to make a difference in the lives of Medicare Advantage plan members throughout Saratoga Countyincluding Saratoga Springs, Clifton Park, and Halfmoon. Enjoy the flexibility to craft your own schedule while educating and supporting members in their homes.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule around your availability
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Conduct comprehensive annual and post-discharge home health risk assessments for Medicare Advantage members
- Evaluate prior and current medical history, medical conditions, and medication histories during each assessment
- Provide patient education regarding ongoing medical conditions and medications
- Identify gaps in care and make clear recommendations for further action or follow-up
- Enjoy a patient-centered approach without developing treatment plans or prescribing medication
Qualifications:
- Active Nurse Practitioner license in New York State
- Board Certification via AANP or ANCC
- Current CPR certification (BLS)
- Minimum of 6 months experience practicing as a Nurse Practitioner preferred
- MSN degree from an accredited nursing program
