Job Description

Deliver vital home health services as an experienced Nurse Practitioner performing risk assessments for Medicare Advantage plan members across San Miguel County, NM, including the communities of Las Vegas, Pecos, and Montezuma. This per diem role offers significant flexibility, allowing you to make a meaningful impact while managing your professional schedule.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Option to create your own schedule
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Perform comprehensive home health risk assessments, generally lasting about 45 minutes per visit
  • Evaluate clients past and present medical histories, current medical conditions, and medication use
  • Educate patients regarding their medical issues and medications
  • Detect and provide recommendations for closing any gaps in care
  • Assignments may include annual health assessments or post-discharge visits
  • No responsibility for developing treatment plans or prescribing medication

Qualifications:

  • Active Nurse Practitioner License in New Mexico
  • Current BLS CPR certification
  • Board Certification through the AANP or ANCC
  • Minimum of 6 months recent NP experience preferred
  • Master of Science in Nursing (MSN) from an accredited institution
