Job Description

Deliver impactful care in a rewarding per diem opportunity as a Nurse Practitioner specializing in home health assessments. This position centers on conducting thorough risk assessments for Medicare Advantage plan members within Grant County, NMincluding locations such as Bayard-Santa Rita, Santa Clara, Tyrone, and Hurley. Each assessment averages approximately 45 minutes and may encompass annual health evaluations or post-discharge visits, tailored to the varying healthcare needs of individuals in their homes.

Schedule:

  • Flexible shifts available all days of the week (MondaySunday)
  • Self-managed schedulechoose your own working days and hours
  • Commitment minimums based on weekly or bi-weekly availability

Key Responsibilities:

  • Perform comprehensive, in-home annual risk assessments for Medicare Advantage members
  • Review and evaluate previous and current medical history, ongoing conditions, and medication regimens
  • Educate patients on existing health conditions and appropriate medication usage
  • Identify care gaps and recommend actionable solutions to optimize patient outcomes
  • No prescriptive authority or ongoing treatment plan development required

Qualifications:

  • Active Nurse Practitioner license in New Mexico
  • MSN from an accredited nursing program
  • Board Certification via AANP or ANCC
  • BLS (CPR) certification required
  • Minimum of six months recent Nurse Practitioner experience preferred

The compensation for this position is $80$90 per home visit, with additional stipends provided for mileage and administrative time.
