Experienced Nurse Practitioners with a passion for home health are invited to make a meaningful impact by conducting in-home risk assessments for Medicare Advantage plan members throughout Seminole County. This per diem opportunity allows for flexibility and autonomy while contributing to better patient outcomes in the community.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own schedule
- Minimum weekly or bi-weekly hour commitment required
Role Overview:
In this position, you will conduct home-based risk assessments for patients living in Sanford, Altamonte Springs, Casselberry, and surrounding communities within Seminole County. Each visit, typically lasting around 45 minutes, may involve an annual health assessment or a post-discharge evaluation. You will review medical histories, assess conditions, and educate patients without the responsibility of prescribing medications or creating treatment plans.
Primary Responsibilities:
- Perform comprehensive risk assessments in members homes
- Evaluate past and current medical history, ongoing conditions, and medication regimens
- Offer patient education on diagnoses and medications
- Identify gaps in care and recommend solutions to support improved health outcomes
- Ensure accurate documentation and thorough communication with care management teams
Qualifications:
- Unencumbered Nurse Practitioner license active in Florida
- Board Certification through AANP or ANCC
- Current BLS (CPR) certification
- MSN from an accredited nursing school
- Minimum 6 months recent experience as a Nurse Practitioner (preferred)
- Strong interpersonal skills and the ability to provide compassionate, one-on-one care
This role offers a competitive per-assessment rate of $80$90, with an additional mileage stipend and compensation for administrative time, providing both meaningful work and financial reward.
