Job Description

Advance your clinical skills by delivering impactful care in a per diem, home-based setting. This role involves conducting home health risk assessments for Medicare Advantage plan members throughout Volusia County. With flexible scheduling, youll manage your own calendar, selecting shifts between Monday and Sunday. Assignments may include visits to Deltona, Daytona Beach, Port Orange, and surrounding locations.

Schedule/Hours:

  • Flexible shifts, Monday through Sunday
  • Set your own schedule
  • Commit to a minimum number of hours weekly or bi-weekly

Key Responsibilities:

  • Perform comprehensive annual health risk assessments during in-home visits, spending approximately 45 minutes with each member
  • Review prior and current medical history, conditions, and medications
  • Provide clear education about health conditions, treatments, and medication regimens
  • Identify potential care gaps and offer recommendations to help improve overall health outcomes
  • No requirement to develop treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in Florida
  • Current CPR certification (BLS)
  • Board certified via AANP or ANCC
  • Minimum of 6 months experience as an NP preferred
  • Master of Science in Nursing (MSN) from an accredited institution
