Make a difference as a Nurse Practitioner specializing in home health risk assessments for Medicare Advantage members in Alamosa County. This per diem opportunity allows you to create your own schedule with flexible shifts between Monday and Sunday, making it ideal for those seeking meaningful work-life balance. Candidates will conduct home visits in communities across Alamosa County, including Alamosa, Blanca, and Hooper, completing each assessment in approximately 45 minutes.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own schedule based on availability
- Requires a minimum weekly or bi-weekly hourly commitment
Responsibilities:
- Perform comprehensive annual health risk assessments in patients homes
- Evaluate medical history, current conditions, and medication regimens
- Provide education on health conditions and proper medication use
- Identify care gaps and recommend solutions for improved outcomes
- No prescribing or development of treatment plans required
Qualifications:
- Active Nurse Practitioner license in Colorado
- Board Certification through AANP or ANCC
- MSN degree from an accredited university
- Current Basic Life Support (BLS/CPR) certification
- Minimum of 6 months of recent Nurse Practitioner experience preferred
Compensation: Earn $80-$90 per evaluation completed, along with a mileage stipend and compensation for administrative time.
