Job Description

Experienced Nurse Practitioners are sought to conduct home health risk assessments for Medicare Advantage plan members throughout Erie County, including Sandusky, Huron, and Vermilion. In this per diem opportunity, each assessment typically takes about 45 minutes and may involve either annual health reviews or post-discharge visits. Practitioners are scheduled in various Erie County locations based on individual shift assignments and patient need.

Schedule:

  • Choose your own shifts between Monday and Sunday
  • Design a schedule that fits your lifestyle
  • Meet minimum weekly or bi-weekly hourly commitment requirements

Key Responsibilities:

  • Complete annual risk assessments in the members home setting for Medicare Advantage members
  • Thoroughly review prior and current medical history, conditions, and all medications
  • Provide education to members regarding medical conditions and medications
  • Identify gaps in care and recommend next steps or follow-up, as appropriate
  • No requirement to create treatment plans or prescribe medications

Required Qualifications:

  • Valid Nurse Practitioner License in Ohio
  • Current CPR certification (BLS)
  • Board Certification: AANP or ANCC
  • Minimum of 6 months of current NP experience is preferred
  • Masters degree in Nursing (MSN) from an accredited institution

This role offers compensation of $80$90 per completed evaluation, in addition to a mileage stipend and administrative time.
