Experienced Nurse Practitioners are needed to conduct comprehensive home health risk assessments on a per diem basis within Bonner County. This role offers a unique blend of flexibility and autonomy, making it ideal for professionals seeking to make a significant impact in community health. You will provide face-to-face health evaluations for Medicare Advantage plan members residing in locations such as Sandpoint, Ponderay, and Priest River.
Schedule:
- Flexible shifts offered Monday through Sunday
- Freedom to customize your schedule to fit your lifestyle
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage plan members in their homes
- Review patients prior and current medical history, chronic conditions, and medication records during each 45-minute visit
- Educate patients on their health conditions, as well as proper use and benefits of prescribed medications
- Identify potential gaps in care and recommend appropriate steps for resolution
- Focus solely on assessments: no treatment planning or medication prescribing required
Qualifications:
- Valid Nurse Practitioner license in Idaho
- Board Certification from AANP or ANCC
- Current Basic Life Support (CPR) certification
- Ideally, six months or more of current NP experience
- Master of Science in Nursing from an accredited institution
