Make a meaningful impact by providing individualized care as a Nurse Practitioner specializing in home health risk assessments. In this per diem role, you will visit Medicare Advantage plan members in Cibola County, NMincluding Grants, Milan, and Acoma Puebloto assess their health and ensure they receive the guidance and support needed for well-being and improved quality of life.

Schedule:

• Flexible shifts available Monday through Sunday
• Create your own schedule around personal commitments
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members in various Cibola County locations
• Review and evaluate prior/current medical history, existing medical conditions, and medication regimens
• Deliver patient education regarding chronic conditions and medication adherence
• Identify and address gaps in care, providing recommendations to patients and care teams for effective resolution
• Note: The position does not require treatment planning or prescribing medications

Qualifications:

• Active, unencumbered Nurse Practitioner license in the state of NM
• Current BLS (CPR) certification
• Board Certification through AANP or ANCC
• Minimum of six months of recent, relevant experience as a Nurse Practitioner is preferred
• MSN degree earned from an accredited nursing school

In this role, you will receive compensation of $80$90 per evaluation, in addition to a mileage stipend and reimbursement for administrative time.
