Step into a vital clinical role conducting home health risk assessments for Medicare Advantage members in the San Marcos and greater Hays County area. This per diem position is perfect for nurse practitioners seeking flexible scheduling and autonomy over their workweek. You will lead comprehensive, in-home evaluations, contributing to improved health outcomes for a diverse member population.
Schedule:
- Flexible shifts available Monday through Sunday

Key Responsibilities:
- Perform in-depth annual home health risk assessments for adult Medicare Advantage members
- Review and evaluate previous and current medical histories, ongoing conditions, and medication lists
- Deliver patient education regarding medical conditions and prescribed medications
- Uncover potential gaps in patient care, providing actionable recommendations for patient and plan follow-up
- Focus exclusively on assessmentsno treatment planning or prescribing involved
Qualifications:
- Active Nurse Practitioner license (TX)
- Board Certification through AANP or ANCC
- Current BLS (CPR) certification
- MSN degree from an accredited program
- At least 6 months of experience as an NP preferred
This position offers a competitive structure: each completed evaluation provides compensation of $80-$90, plus a mileage stipend and pay for administrative documentation time.
