In this dynamic role, you'll perform home health risk assessments throughout various communities, providing valuable insights into the health and wellness of Medicare Advantage plan members. You'll visit individuals in their homes, with assessment types that may include annual health evaluations and post-discharge visits. Each assessment typically takes about 45 minutes, allowing for thorough care and thoughtful patient education. Service areas include Concord, Kannapolis, Locust, and surrounding regions in Cabarrus County, ensuring a varied and rewarding patient population.

Schedule:

  • Flexible shifts offered seven days a week
  • Choose your own schedule
  • Minimum weekly or bi-weekly commitment, tailored to your availability

Key Responsibilities:

  • Complete comprehensive annual home health risk assessments for Medicare Advantage members
  • Review medical histories, conditions, and medication usage
  • Educate patients on health maintenance, medical conditions, and medications
  • Identify care gaps and provide actionable recommendations (no prescription or treatment planning required)

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Board certification via AANP or ANCC
  • Current BLS/CPR certification
  • Minimum of six months current NP experience preferred
  • MSN from an accredited nursing program
