Contribute your clinical expertise as a Nurse Practitioner specializing in home health risk assessments. This per diem position offers a flexible schedule where you can create your own shifts, working between Monday and Sunday with the option to commit to a minimum number of weekly or bi-weekly hours. Assignments will involve visits within Polk County, including Salem and surrounding areas.

Responsibilities include:

• Conducting annual risk assessments for Medicare Advantage plan members in their homes
• Completing evaluations of past and current medical history, medical conditions, and medications
• Educating patients on their medical conditions and prescribed medications
• Identifying gaps in care and making recommendations for closing these gaps
• Engaging members through annual health assessments or post-discharge visits
• No requirement for creating treatment plans or prescribing medications

Qualifications:

• Valid Nurse Practitioner license in Oregon
• Current CPR certification (BLS)
• Board Certification through AANP or ANCC
• MSN degree from an accredited nursing program
• At least 6 months of current Nurse Practitioner experience preferred
