Advance your career in a per diem role focused on delivering quality home health services as a Nurse Practitioner. In this dynamic position, you will conduct comprehensive home health risk assessments across various locations throughout Mahoning County, including Youngstown, Boardman Township, and Austintown Township. Each evaluation, typically lasting around 45 minutes, may range from annual health assessments to post-discharge visits for Medicare Advantage plan members, providing essential healthcare insights and recommendations without the responsibilities of prescribing or developing treatment plans.

Key Responsibilities:

• Perform thorough annual health risk assessments in patient homes for Medicare Advantage members
• Assess medical histories, ongoing health conditions, and medication use
• Offer patient education on managing medical issues and medication adherence
• Identify gaps in care and recommend solutions for optimal patient outcomes
• No requirement for prescribing medications or developing treatment plans

Qualifications:

• Valid Nurse Practitioner License in Ohio
• Board certification through AANP or ANCC
• Graduate of an MSN program accredited by an approved nursing school
• Current CPR (BLS) certification
• At least 6 months of recent NP experience preferred

This opportunity offers highly competitive compensation, with payment ranging from $80 to $90 per completed evaluation. Additionally, youll benefit from a mileage stipend and paid administrative time.
