Job Description

Deliver impactful care to patients in their homes through flexible, per diem work as a Nurse Practitioner focused on home health risk assessments. Schedule:

  • Flexible shifts available seven days a week
  • Ability to create your own schedule
  • Minimum weekly or bi-weekly hour commitment

Key Responsibilities:

  • Conduct comprehensive home health risk assessments for Medicare Advantage plan members in Salem and throughout Columbiana County
  • Evaluate patients prior and current medical histories, medical conditions, and medication regimens during each visit, averaging 45 minutes per assessment
  • Educate patients about their health and medications, fostering a deeper understanding of their conditions and treatment
  • Identify gaps in care and provide clear recommendations to improve patient outcomes
  • Visits may involve annual health assessments or post-discharge evaluations
  • No requirement for prescribing medication or forming treatment plans, allowing you to focus on assessment and education

Qualifications:

  • Current Nurse Practitioner license in Ohio
  • Board Certification through AANP or ANCC
  • Current CPR certification (BLS)
  • MSN degree from an accredited nursing school
  • At least 6 months of recent Nurse Practitioner experience is preferred
