Contribute your expertise as a Nurse Practitioner by delivering essential home health risk assessments to Medicare Advantage plan members across Rockingham County. Schedule:

• Flexible shifts available Monday through Sunday
• Control your own schedule to fit your availability
• Minimal weekly or bi-weekly hourly commitment required

What You'll Do:

• Conduct comprehensive, 45-minute home health risk assessments in patient residences, potentially as part of annual assessments or post-discharge visits
• Review and evaluate each individuals current and past medical history, medical conditions, and medication usage
• Educate patients on their health conditions and medications, promoting ongoing self-care and adherence
• Identify care gaps and provide recommendations to support improved health outcomes
• No responsibility for treatment plans or prescribing medicationsfocusing exclusively on assessment and education

Required Qualifications:

• Active Nurse Practitioner license in New Hampshire
• Board Certification via AANP or ANCC
• Current CPR (BLS) certification
• MSN from an accredited nursing program
• At least six months of recent Nurse Practitioner experience (preferred)
• Commitment to patient-centered care and reliable, independent work

Compensation and Perks: Earn $80-$90 per evaluation, along with a mileage stipend and compensation for administrative time.
