Job Description

Experienced Nurse Practitioners are sought for a flexible per diem opportunity conducting home health risk assessments for Medicare Advantage plan members in Marion County, Illinois. This is an excellent fit for clinicians seeking autonomy and control over their schedule with visits taking approximately 45 minutes each, occurring in patients homes throughout locations such as Centralia, Salem, and Sandoval.

Schedule:

  • Flexible shifts available seven days a week (MondaySunday)
  • Self-schedulingchoose shifts that work best for you
  • Minimum commitment of a set number of hours per week or bi-weekly

Key Responsibilities:

  • Complete annual and post-discharge home health risk evaluations for Medicare Advantage members
  • Review and document patients medical histories, co-morbidities, and medication regimens
  • Educate patients about their health conditions and medications
  • Recognize and identify care gaps and make actionable recommendations to improve outcomes
  • No need to develop ongoing treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in the state of Illinois
  • MSN degree from an accredited nursing school
  • National board certification (AANP or ANCC)
  • Current BLS (CPR) certification
  • At least 6 months of recent nurse practitioner experience preferred

Successful candidates will conduct comprehensive, patient-centered assessments while enjoying the flexibility of per diem scheduling. Compensation includes $80$90 per evaluation, with additional stipends for mileage and administrative time, making this a rewarding option for independent and compassionate clinicians.
