Job Description
Experience the impact of direct patient care as you perform comprehensive home health risk assessments for Medicare Advantage members throughout Pinellas County. This per diem Nurse Practitioner opportunity offers the flexibility of self-scheduling, with shifts available seven days a week and the ability to tailor your availability. Each assessment takes approximately 45 minutes and may vary in natureranging from annual health evaluations to post-discharge visits.
Schedule:
- Flexible shifts Monday through Sunday
- Design your own schedule
- Commit to your preferred weekly or bi-weekly hours
Key Responsibilities:
- Perform thorough home health risk assessments for Medicare Advantage enrollees
- Review and evaluate patients medical and medication histories
- Educate individuals on health conditions and proper medication usage
- Identify and communicate gaps in care, offering recommendations for follow-up
- Note: No treatment plans or prescribing requiredfocus is on assessment and education
Qualifications:
- Active Nurse Practitioner license in Florida
- Board Certification from AANP or ANCC
- Current BLS/CPR certification
- Master of Science in Nursing (MSN) from an accredited institution
- Minimum of 6 months experience as a Nurse Practitioner preferred
This position provides a competitive per-evaluation rate between $80 and $90, in addition to a mileage stipend and compensation for administrative time.
