Job Description

Experienced Nurse Practitioners are needed to conduct in-home health risk assessments on Medicare Advantage plan members. These assessments serve as annual wellness checks or post-discharge visits and are typically completed in approximately 45 minutes. Service areas may include various locations within Pinellas County, such as St. Petersburg, Clearwater, and Largo. This per diem opportunity is ideal for professionals who value autonomy and flexibility in their schedules.

Schedule:

  • Flexible shifts available between Monday and Sunday
  • Ability to create your own schedule
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Perform comprehensive annual home health risk assessments for Medicare Advantage members
  • Review and evaluate prior and current medical histories, conditions, and medication use
  • Educate patients regarding their medical conditions and medication management
  • Identify and report any gaps in care, providing recommendations for improvement
  • No requirement for creating treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in Florida
  • Current Basic Life Support (BLS) certification
  • Board certification through AANP or ANCC
  • A minimum of six months' recent NP experience is preferred
  • Master of Science in Nursing (MSN) from an accredited institution

Nurse Practitioners participating in this opportunity will receive $80-$90 per evaluation, along with a mileage stipend and additional compensation for administrative time spent.
