Nurse Practitioners skilled in Home Health are sought for a per diem opportunity providing in-home risk assessments for Medicare Advantage plan members throughout St. Charles County, MO. This role offers autonomy and flexibility, allowing professionals to create their own schedules and select shifts between Monday and Sunday. Assessments take place in patients homes in areas such as OFallon, St. Charles, and St. Peters. Each visit lasts approximately 45 minutes and may include either annual health assessments or post-discharge evaluations, with no treatment planning or prescribing required.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own work schedule
• Minimum commitment required on a weekly or bi-weekly basis

Responsibilities:

• Perform comprehensive home health risk assessments for Medicare Advantage members
• Thoroughly review and evaluate medical histories, current conditions, and medications
• Educate patients regarding their medical conditions and prescribed therapies
• Detect care gaps and provide clear, actionable recommendations to resolve them
• Focus on education and care coordinationno treatment planning or prescribing

Qualifications:

• Active Nurse Practitioner License in Missouri
• Valid CPR certification (BLS)
• National Board Certification (AANP or ANCC)
• Minimum 6 months of NP experience preferred
• Master of Science in Nursing (MSN) from an accredited program

Compensation for this per diem position is $80$90 per evaluation, with an additional mileage stipend and compensation for administrative time.
