Job Description

Embark on a rewarding per diem opportunity as a Nurse Practitioner specializing in Home Health. In this role, you will perform home-based risk assessments for Medicare Advantage plan members throughout Robeson County, including communities such as Lumberton, Red Springs, and Pembroke. Each home visit, lasting around 45 minutes, may involve either an annual health assessment or a post-discharge evaluation. With a minimum weekly or bi-weekly hourly commitment and the flexibility to design your own schedule, you can truly balance professional commitments with your lifestyle.

Schedule:

  • Flexible shifts available seven days a week, Monday through Sunday
  • Set your own schedule
  • Minimal weekly or bi-weekly hour requirements for added flexibility

Key Responsibilities:

  • Complete home health risk assessments for Medicare Advantage patients
  • Review new and prior medical history, current conditions, and medication usage
  • Educate patients on their conditions and treatments during visits
  • Identify and communicate gaps in care, offering solutions to facilitate patient well-being
  • Note: This position does not involve prescribing medications or creating formal treatment plans

Qualifications:

  • Current Nurse Practitioner license in North Carolina
  • Active CPR certification (BLS)
  • Board Certification via AANP or ANCC
  • MSN degree from an accredited institution
  • At least 6 months of recent NP experience (preferred)
