This per diem opportunity allows for a highly flexible schedule, empowering you to choose when and where you work throughout Perry County, MOincluding Perryville, Altenburg, and Frohna. Enjoy creating your own shifts between Monday and Sunday, with the ability to tailor your weekly or bi-weekly commitment.
Schedule:
- Flexible shifts available Monday through Sunday

- Choose a minimum hourly commitment weekly or bi-weekly
Essential Responsibilities:
- Conduct comprehensive annual home health risk assessments for Medicare Advantage health plan members.
- Review and evaluate patients prior and current medical and medication histories.
- Provide clear education to patients regarding their medical conditions and prescribed medications.
- Identify gaps in care and offer recommendations to enhance patient outcomes.
- No requirement for developing treatment plans or prescribing medications.
Desired Qualifications:
- Current Nurse Practitioner license valid in MO.
- Active CPR certification (BLS).
- National board certification (AANP or ANCC).
- MSN degree from an accredited nursing program.
- At least 6 months of experience as a Nurse Practitioner is preferred.
