In this per diem role, you'll visit Medicare Advantage plan members throughout Leon County, including Tallahassee, Woodville, and Midway, to perform comprehensive health evaluations in the comfort of their homes.

Schedule:

• Flexible shifts available seven days a week
• Design your own schedule to fit your lifestyle
• Commit to a minimum number of hours weekly or bi-weekly

Key Responsibilities:

• Conduct thorough annual home health risk assessments for a Medicare Advantage health plan
• Review and evaluate patients' medical history, current conditions, and medication use
• Educate patients on their health conditions and prescribed medications to promote understanding and compliance
• Identify care gaps and make recommendations for follow-up or intervention
• No requirement for creating treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in Florida
• Board certification through AANP or ANCC
• Current CPR certification (BLS)
• Completion of an MSN degree from an accredited nursing program
• At least 6 months of NP experience is preferred
