Job Description

Deliver high-quality home health evaluations for Medicare Advantage plan members as a Nurse Practitioner. This per diem opportunity will have you conducting risk assessments throughout Duval County, including Jacksonville, Jacksonville Beach, and Atlantic Beach. The typical home visit lasts approximately 45 minutes and may include either annual health assessments or post-discharge visits. The role is ideal for professionals seeking flexible work and varied field experience without ongoing treatment planning or prescription responsibilities.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own work schedule
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Complete annual home health risk assessments in members' homes
  • Review and evaluate patients medical history, current conditions, and medication records
  • Offer clear education regarding medical conditions and medication regimens
  • Identify gaps in care and provide actionable recommendations
  • Focus exclusively on evaluationsno treatment plans or medication prescribing involved

Qualifications:

  • Licensed Nurse Practitioner in Florida
  • Current CPR certification (BLS)
  • Board Certified through AANP or ANCC
  • Minimum 6 months of recent NP experience preferred
  • MSN from an accredited nursing program
