Job Description

This per diem opportunity offers the flexibility to craft your own work schedule, with shifts available any day of the week to best suit your preferred work-life balance.

Schedule:

  • Flexible shifts between Monday and Sunday
  • Self-selected scheduling
  • Minimum weekly or bi-weekly hourly commitment

Responsibilities:

  • Perform comprehensive annual health risk assessments in member homes
  • Review and assess both past and present medical histories, conditions, and medications
  • Educate individuals on management of their conditions and proper medication usage
  • Identify gaps in care or preventive services, making recommendations to address these needs
  • Visits may include annual assessments or post-discharge transitions
  • No prescribing or treatment plan creation required

Qualifications:

  • Valid Nurse Practitioner license in Florida
  • Active CPR certification (BLS required)
  • Board certification by AANP or ANCC
  • At least 6 months of Nurse Practitioner experience preferred
  • MSN degree from an accredited program

You'll be compensated at $80-$90 for each evaluation completed, with an additional mileage stipend and reimbursement for approved administrative time.
