Advance your career as a Nurse Practitioner specializing in home health risk assessments across Osceola County, FL. Engage directly with Medicare Advantage members residing in communities including Kissimmee, St. Cloud, and Poinciana. This per diem opportunity offers the flexibility to craft your own work schedule, with shifts available any day of the week to best suit your preferred work-life balance.
Schedule:
- Flexible shifts between Monday and Sunday
- Self-selected scheduling
- Minimum weekly or bi-weekly hourly commitment
Responsibilities:
- Perform comprehensive annual health risk assessments in member homes
- Review and assess both past and present medical histories, conditions, and medications
- Educate individuals on management of their conditions and proper medication usage
- Identify gaps in care or preventive services, making recommendations to address these needs
- Visits may include annual assessments or post-discharge transitions
- No prescribing or treatment plan creation required
Qualifications:
- Valid Nurse Practitioner license in Florida
- Active CPR certification (BLS required)
- Board certification by AANP or ANCC
- At least 6 months of Nurse Practitioner experience preferred
- MSN degree from an accredited program
You'll be compensated at $80-$90 for each evaluation completed, with an additional mileage stipend and reimbursement for approved administrative time.
