As a Nurse Practitioner specializing in home health, youll play a vital role in improving patient outcomes by conducting focused risk assessments for Medicare Advantage plan members. This per diem position offers remarkable flexibilityadapt your shifts to fit your lifestyle with coverage needed any day of the week. Youll conduct in-home assessments throughout St. Louis County, including areas such as Florissant, Chesterfield, and Wildwood. Each assessment typically takes about 45 minutes and may include annual wellness evaluations or post-discharge visits.

Schedule:

• Flexible shifts available Monday through Sunday
• Create your own schedule
• Minimum weekly or bi-weekly hourly commitment required

Core Responsibilities:

• Perform comprehensive home health risk assessments for Medicare Advantage members
• Review prior and current medical histories, ongoing conditions, and medication usage
• Deliver patient education on chronic health conditions and pharmacological therapies
• Identify care gaps, provide recommendations, and document findings
• No requirement to create formal treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in Missouri
• Current CPR certification (BLS)
• Board Certification via AANP or ANCC
• MSN degree from an accredited nursing program
• At least 6 months of Nurse Practitioner experience preferred

You will be compensated $80-$90 for each completed evaluation, plus receive a mileage stipend and reimbursement for administrative time.
