Job Description

Make a meaningful impact by providing essential healthcare services in a home health setting. This per diem opportunity centers on conducting home health risk assessments for Medicare Advantage members within St. Charles County. As a Nurse Practitioner, youll support patients across locations in O'Fallon, St. Charles, and St. Peters, playing an integral role in evaluating and educating individuals in the comfort of their homes.

Schedule:

  • Flexible shifts available any day of the week (MondaySunday)
  • Design your own work schedule
  • Commitment required on a weekly or bi-weekly basis (minimum hourly commitment applies)

Key Responsibilities:

  • Perform annual health risk assessments for Medicare Advantage members
  • Review and document prior and current medical history, ongoing health conditions, and medications
  • Provide clear patient education regarding conditions and medication management
  • Identify care gaps and offer practical recommendations to support optimal health outcomes
  • No requirement to develop treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in Missouri
  • Graduated with an MSN from an accredited nursing program
  • Current CPR certification (BLS)
  • Board certification through AANP or ANCC
  • At least six months of recent experience as an NP preferred
