Minimum weekly or bi-weekly hourly commitments enable you to tailor your workload to your lifestyle and other professional responsibilities.
You will conduct detailed annual health risk assessments for Medicare Advantage plan members in their homes, ensuring personalized, quality care in O'Fallon, St. Charles, and St. Peters. Each home visit typically lasts about 45 minutes and may include annual health evaluations or post-discharge checkups.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule
- Minimum hourly commitment on a weekly or bi-weekly basis
Responsibilities:
- Perform comprehensive home health risk assessments for Medicare Advantage members
- Review and evaluate patients' prior and current medical conditions, histories, and medications
- Educate patients about their conditions and medications
- Identify gaps in care and provide actionable recommendations for follow-up
- Note: No treatment plan development or medication prescribing required
Qualifications:
- Current Nurse Practitioner license in Missouri
- CPR certification (BLS)
- Board certification through AANP or ANCC
- MSN from an accredited school of nursing
- Preferred: at least 6 months of NP experience
This position offers a pay range of $80$90 per evaluation, including a mileage stipend and compensation for administrative time.
