Enhance patient outcomes while enjoying flexible, per diem work as a Nurse Practitioner specializing in home health risk assessments. Take charge of your schedule and support Medicare Advantage members in Wood County, conducting visits in locations such as Parkersburg, Vienna, and Williamstown. Each assessment averages 45 minutes and may center around annual wellness or post-discharge evaluations.
Schedule:
- Flexible shifts available seven days a week
- Create your own work schedule, accommodating minimum weekly or bi-weekly commitments
Primary Responsibilities:
- Conduct thorough annual risk assessments for Medicare Advantage plan members in their homes
- Review patients current and previous medical and medication histories
- Assess for chronic and acute health conditions
- Deliver patient-centered education about diagnoses and medication regimens
- Pinpoint gaps in care and recommend strategies for closing them
- Focus exclusively on evaluationno need to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in West Virginia
- Board Certification through AANP or ANCC
- Current BLS/CPR certification
- Minimum of six months recent experience as a Nurse Practitioner is preferred
- MSN degree from an accredited nursing program
Compensation: Earn between $80 and $90 for each evaluation, with additional mileage stipend and compensation for administrative time.
