Job Description

Take advantage of a rewarding per diem opportunity designed for Nurse Practitioners experienced in home health care. In this key position, youll perform home health risk assessments for Medicare Advantage plan members. Each evaluation, typically lasting 45 minutes, can include annual health assessments or post-discharge visits. Assessments are conducted in the homes of patients residing in Charleston and the greater Mercer County area, including South Charleston and St. Albans. Schedules are highly flexible, allowing you to create your own shifts between Monday and Sunday with a minimum weekly or bi-weekly hourly commitment.

Key Responsibilities:

  • Perform comprehensive home health risk assessments for Medicare Advantage members
  • Review medical history, current medical conditions, and medication history
  • Educate members on their health conditions and prescribed medications
  • Identify any gaps in care and provide actionable recommendations
  • No requirement for developing treatment plans or prescribing medications

Schedule/Hours:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule
  • Option for minimum weekly or bi-weekly hourly commitments

Qualifications:

  • Active WV Nurse Practitioner License
  • Board Certification through AANP or ANCC
  • Current CPR certification (BLS)
  • MSN from an accredited nursing program
  • Minimum of 6 months current NP experience preferred

This per diem role offers attractive compensation: $80-$90 per evaluation, plus a mileage stipend and administrative time. You'll have the freedom to control your schedule while delivering critical assessments that help improve patient outcomes.
