Job Description
Take on an impactful per diem opportunity as a Nurse Practitioner specializing in home health, conducting risk assessments for Medicare Advantage members across Bonner County, including Sandpoint, Ponderay, and Priest River. In this role, youll provide vital care and assessments in patients homes, allowing for both a flexible and rewarding work environment.
Schedule:
- Flexible shifts available seven days a week
- Design your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct comprehensive annual health risk assessments during home visits, each typically lasting around 45 minutes
- Evaluate patients current and previous medical histories, health conditions, and medication regimens
- Educate patients regarding their health conditions and medication use
- Identify care gaps and supply recommendations to optimize patient care
- Focus solely on assessments and educationno prescribing or treatment plan creation required
Desired Qualifications:
- Active Nurse Practitioner license in Idaho
- Board certification through AANP or ANCC
- Current CPR certification (BLS)
- MSN degree from an accredited nursing school
- Minimum of six months recent NP experience is preferred
