This role offers the opportunity to visit Medicare Advantage plan members throughout Androscoggin County, MEincluding areas such as Lewiston, Auburn, Lisbon, and Poland. With the flexibility to create your own schedule, youll play a vital part in promoting preventive care and improving patient outcomes in a one-on-one setting.

Schedule:

• Flexible shifts available Monday through Sunday
• Self-scheduling to fit your personal commitments
• Minimum weekly or bi-weekly hour commitments

Key Responsibilities:

• Conduct comprehensive annual health risk assessments for Medicare Advantage members in patient homes
• Review prior medical history and current health conditions, as well as medication usage
• Educate patients on their health concerns and treatment regimens
• Identify gaps in care and offer appropriate recommendations
• No prescribing medications or creating treatment plans required
• Complete accurate clinical documentation following each assessment

Qualifications:

• Active Nurse Practitioner license in Maine
• Current BLS/CPR certification
• Board Certified through either AANP or ANCC
• MSN from an accredited nursing school
• Minimum of 6 months of current NP experience (preferred)
