A dynamic opportunity is available for a Nurse Practitioner with home health and risk assessment experience to provide per diem services across Rutherford County. This role involves conducting in-person home health risk assessments for Medicare Advantage members in areas like Forest City, Spindale, and Rutherfordton. Each assessment typically lasts about 45 minutes and may involve annual health reviews or post-discharge visits.

Core Responsibilities

  • Complete annual health risk assessments in member homes
  • Review medical history, current conditions, and medications
  • Deliver education on disease processes and medication management
  • Identify care gaps and recommend steps for improvement
  • Direct treatment planning and prescribing are not required

Qualifications

  • Active North Carolina Nurse Practitioner license
  • Board certification (AANP or ANCC)
  • Current CPR (BLS) certification
  • MSN degree from an accredited nursing program
  • At least 6 months of NP experience preferred
